CenterWell Home Health specializes in personalized, comprehensive in-home care for patients managing a chronic condition or recovering from injury, illness, surgery, or hospitalization. Our care teams include nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers – all working together to help patients regain their independence and live healthier lives.
OneHome, part of the broader CenterWell Home Solutions, coordinates a full range of post-acute care, including home health, infusion therapy and durable medical equipment services at patients’ homes. OneHome’s patient focused, value-based model creates one integrated point of accountability that coordinates with physicians, hospitals and health plans serving more than one million health plan members nationwide.
CenterWell Pharmacy provides millions of customers with safe, reliable and affordable pharmacy care through our mail-order delivery, retail pharmacy locations, over-the-counter products and specialty pharmacy services. CenterWell Pharmacy promotes medication adherence through strong clinical programs that make medication management simple. Data indicates CenterWell Pharmacy users have a higher increase in medication adherence of up to 2.7% for members with specific hyperlipidemia, diabetes or hypertension compared to non-CenterWell pharmacy users. CenterWell Pharmacy users also have up to 190 fewer admits per thousand on average for members with hypertension, hyperlipidemia or diabetes.1
For patients facing certain chronic and/or complex medical conditions, CenterWell Specialty Pharmacy provides additional support. For patients with cancer, published data indicates that use of CenterWell Specialty Pharmacy’s clinical programs was associated with 14.2% fewer acute hospitalizations versus users of other pharmacies. Patients engaged with dedicated clinicians in the same study had a 19.9% reduction in medical costs.2
